Q: Who can register to join the community?
A: Registrants must be 18 years or older and living in the U.S.
Q: How do I sign up to join?
A: On the home page of www.yourcaremoments.com, click on the button entitled "Click here to register". This will navigate you to the registration questions. You will be asked to provide your full name, email address, date of birth, home zip code and your medical conditions. You can select up to five medical conditions for your registration. Please only select the medical conditions that you were diagnosed with. If you only have one, only fill out one. Once you complete the registration questions, an email will be sent your your email address. Make sure to open this email and click the confirm email address button to activiate your registration. IF YOU DO NOT CONFIRM YOUR EMAIL ADDRESS, YOU WILL NOT BE ABLE TO JOIN THE COMMUNITY.
Q: How do I log in?
A: On the home page of www.yourcaremoments.com, enter in your email address and password. Your username is your registered e-mail address. Press the login button to login.
Q: What if I forget my password?
A: Click the reset password tab on the top of the page. Enter in your email address and hit the "Create New Password" button. This will send an automated email to your inbox. Hit the "Recover Password" button in your email. This will ask you to type in your new password and then confirm your new password. Hit the "Reset Password" button. If you have any trouble with this, you can email email@example.com or fill out our Contact Us form on the website to assist you.
Q: How do I update my profile?
A: To update your profile, you must first log in. Once you are logged in, click on the "My Profile" tab. You will be able to update your Screen Name, Time Zone, E-mail address, Password, Opt-in status, and invitations preference. Once you make your updates, press the "Submit" button on the bottom of the page.

1. How do I earn points and rewards?
Participants may earn points by engaging in various activities approved by Zitter Health Insights ("ZHI"), such as participating in a survey panel, contest, or promotion, participating in our “refer-a-friend” program, or completion of surveys or questionnaires ("Approved Activity"). Each Approved Activity earns a certain predetermined number of points. Points are redeemable for various rewards, such as gift cards, vouchers, cash payments via PayPal, and donations to charitable organizations ("Rewards").
2. How can I redeem points for rewards?
Points can be redeemed for Rewards through ZHI's Rewards program, available at www.yourcaremoments.com/rewards. Except in the context of redeeming Rewards, points have no cash or other value. All Rewards redemptions are final. Please note that additional terms and conditions may apply in connection with the certain Rewards. ZHI cannot replace any lost, stolen, misplaced, or damaged Rewards.
If a participant elects to redeem a Reward for a donation to an approved charity, ZHI will make a cash donation in the amount specified for the Reward that is redeemed. Participants cannot claim ZHI’s donation as a charitable donation or receive any tax benefit as a result of the redemption of such a Reward.
ZHI uses reasonable efforts to ensure that points are credited and debited appropriately; however, it is each participant’s responsibility to regularly review their account to ensure that all transactions have been recorded correctly. 3. Do points expire?
Points do not expire but may be cancelled or voided if a participant terminates his or her account, or is suspended by ZHI. Any participant who wishes to cancel his or her account, should redeem all points available for redemption for Rewards prior to closing the account.
4. Do I have to pay taxes on Rewards that I receive?
ZHI cannot provide any legal or tax advice to participants. Participants are encouraged to seek professional advice regarding their tax and other legal obligations.
Please note that ZHI may provide appropriate government agencies and taxing authorities with information related to your Rewards redemption history, and if required, ZHI may withhold taxes in connection with any payments, incentives or other Rewards earned in connection with Approved Activities. Each participant agree to fully cooperate with and assist ZHI in complying with its reporting or withholding obligations.
5. Are points or Rewards transferrable?
No. Participants may not transfer, assign, or pledge points or Rewards.
6. What else should I know about ZHI’s rewards program?
ZHI reserves the right to modify or change the terms and conditions of our Rewards program, or discontinue it entirely for any reason, at any time.

Q: How do I check and redeem rewards?
A: To check and redeem your rewards, you must first log in. Once you are logged in, hover over the "My Profile" tab. A dropdown of choices will pop up. Click on the "My Rewards" option. This page will show you how many available points you have at the top of the page. You will also see your different options to redeem your points. Click the option that you would like to redeem your points with. Note: You must earn at least 50 points in order to redeem your rewards. Redeem requests can take up to 5 business days to process. If you have any trouble with this, you can email email@example.com or fill out our Contact Us form on the website to assist you.
Q: What are my rewards options?
A: Community members can redeem their rewards through Tango Gift Cards and Amazon Gift Cards. You must earn at least 50 points in order to redeem your rewards.
Q: How do I participate in surveys?
A: In order to qualify for surveys, you must first register with YourCareMoments. Once you complete the registration, you wil receive an email asking you to confirm your email address. IF YOU DO NOT CONFIRM YOUR EMAIL ADDRESS, YOU WILL NOT BE ABLE TO PARTICIPATE IN SURVEYS. Once you confirm your email address, you will have the opportunity to receive surveys from YourCareMoments. When new surveys are available for you, you will receive an e-mail inviting you to participate in the survey. You can also check for available surveys by logging into www.yourcaremoments.com and check your available surveys. To particiapate, click on the survey link provided for you. Remember, you will not always qualify to participate in every survey available to you.
Q: Where do I find my available surveys on the website?

Q: How long are the surveys?
A: Each survey opportunity will vary in length, however, most of the surveys range from 10 - 15 minutes long. You will be notified of the survey length in your survey invitation.
Q: Who is YourCareMoments and what do they do?
A: YourCareMoments is a project of Zitter Health Insights, a specialized business intelligence firm that provides insights to biopharmaceutical manufacturers and health insurance companies related to the affordability of and access to medications. Simply put, we provide healthcare data to help improve access to medications. Since 1989, Zitter Health Insights has worked with biopharmaceutical manufacturers and managed care organizations to develop effective access and reimbursement strategies by combining expertise in economics, medicine, the life sciences, and marketing. Q: Is YourCareMoments contracted with any pharmaceutical companies?
A: YourCareMoments works with pharmaceutical companies, but we are not working directly for any specific company or drug.
Q: What does YourCareMoments do with the survey research?
A: The research is received and analyzed by Zitter Health Insights. The data is then sold to Bio-Pharmaceutical companies to help them gain insight to patient healthcare experiences, opinions and habits.
Q: Will YourCareMoments sell my email address?
A: YourCareMoments will never sell or share patient email addresses. We ensure that all of the information our participants share with us is safe.
